1)
SPONSOR: The Promotion is sponsored by Dana-Farber Cancer
Institute, d/b/a The Jimmy Fund, 44 Binney St., Boston, MA 02215, with offices
at 10 Brookline Place West, Brookline, MA, 02445. Upon request,
a copy of the last filed annual report of The Jimmy Fund may be obtained from
Dana- Farber Cancer Institute.

HOW TO PARTICIPATE (NO
PURCHASE REQUIRED): Obtain a Jimmy
Fund Value Card with a Llucky
Nnumber at participating Jiffy Lube
shops (located in MA, NH, RI & VT) between October 31, 2005 and December
31, 2005 (while supplies last). You may also obtain a Value Card with a Llucky Nnumber without purchase or
contribution (while supplies last) by sending a self addressed stamped envelope
to, "Jiffy Lube/Jimmy Fund Value Card Lucky Number
promotion request, The Jimmy Fund (Attn. S. Berger), 10 Brookline Place
West, Brookline, MA 02445. Mail-in requests must be received by December 31,
2005 to be honored. One request per envelope, each request mailed separately. Vermont
residents may omit return postage for mail-in requests,
including but not limited to, a Value Card with a Llucky Nnumber and Official
Rules Request.

HOW TO PLAY ONLINE: Jiffy Lube Value Card Lucky Number web site, www.jimmyfund.org/sweeps will be
available for participants to check their Value Card Lucky Number alphanumeric
code starting at 12:01 AM8:00AM on
October 3112, 2005, and ending 11:59 PM on
January 9, 2006.
The Value Card Lucky Number (6-digit alphanumeric
code) may be found on the right side of the Value Card. Enter this information
in the area indicated on the online website game page, in the exact order as found
on the Value Card. If the alphanumeric code matches in the exact same order as
one of the combinations that has been randomly pre-selected as a winning
combination, you will instantly be notified that you are a potential Prize
Winner (subject to verification of eligibility),. YOU WILL NEED THE BOTTOM PORTION OF YOUR
VALUE CARD SHOWING YOUR LUCKY NUMBER TO CLAIM YOUR PRIZE!.
Your code will be recorded in the exact order entered/played. The randomly pre-selected winning combinations are
the ones stored in the COHEN-FRIEDBERG ASSOCIATES database only. Each winning
combination must be matched on a single Value Card and submitted for
verification. Letters and numbers cannot be combined from any other Value Card
number combination submitted by entrants.
If you cannot access the internet to check your
Lucky Number code, you may mail it to the following address with a written
request for confirmation: Jimmy Fund Value Card Confirmation Request, PO Box
2001, Framingham, MA 01703-2001. Your written request should include your name,
mailing address and phone number, but you will only be contacted if your Lucky
Number code is a winner. Please retain a photocopy, for your records, of any
Lucky Nnumber confirmation request mailed to
the confirmation address above. It is recommended that you send your potential
winning Valuegame Ccard via Registered or Certified Mail,
Return Receipt Requested. Proof of mailing is not proof of receipt. Requests
must be received by January 16, 2006. Any confirmation request(s) received by
mail after January 16, 2006, will automatically become null and void.

HOW TO VERIFY AND REDEEM
AN ONLINE WINNING VALUE CARD: All
prize-winning claims for the online portion of the promotiongame must be mailed to the Fulfillment
Center address below for verification. You are not a winner until your Value Card Lucky Nnumber has been verified and you have
complied with all terms of these Official Rules. Do not present your Valuegame Ccard for any potential online winning
combination to any Jiffy Lube personnel, as they are not authorized to verify
or redeem these online prize claims.
To claim a potential online winning combination,
remove the Lucky Number portion of your Value Card and sign the front of the
original Lucky Number portion of the Value Card. On a separate piece of paper,
clearly print your name, complete address, city, state and zip code and the
prize you claim to have won. IMPORTANT: Please retain a photocopy, for your records, of any Lucky Nnumber submission mailed to the
Fulfillment Center address below. It is recommended that you send your potential
winning Valuegame Ccard via Registered or Certified Mail,
Return Receipt Requested. Proof of mailing is not proof of receipt.

All prize claims must be received by January 16, 2006. Any prize claim(s) received by mail after January
16, 2006, will automatically become null and void. If the grand Grand Prize
winners cannot be contacted, provides
incorrect address, is ineligible, fails to return the completed and executed
Affidavit of Compliance and Release Form, the prize will not be awarded and an
alternate winner will be selected in a second chance drawing (see rule 69).

SECOND CHANCE DRAWING: If any of the online prizes are remain unclaimed and
not awarded, a random drawing will be conducted to award remaining prizes. To
enter the second chance drawing: write your name, complete address, and
telephone number on a post card and mail to: Jimmy Fund Value Card Drawing, PO
Box 2242, Framingham, MA 01703-2242. All entries must be received by January 9,
2006. Drawing to be held on or about January 28, 2006. Odds of winning depend
on the number of prizes remaining unclaimed and the number of valid entries
received.

assigned an alphanumeric code. All odds are based on a distribution of 75,000
Value Cards. Prizes not won or claimed will become void and not awarded in any
Second Chance Drawing.

CONDITIONS OF
PARTICIPATION: By participating in
this promotion, the participant agrees to assume all responsibility for the
submission of potential winning Value Card Lucky Numbers to the Fulfillment
Center. Prize claimant assumes all responsibility for mailing potential winning
Lucky Number Value Cards to the correct address for verification. Prizes will
not be awarded if potential winning cards are submitted in a manner
inconsistent with the instructions contained in these Official Rules. All Valuegame Ccards and codes are subject to
verification by COHEN-FRIEDBERG ASSOCIATES, an independent judging organization
whose decisions are final and binding as they pertain to this promotiongame. Claims Game materials are automatically null and void and will
be rejected if prize claim submissions are lost, stolen, late, mutilated,
misdirected, illegible, incomplete, photocopied, mechanically reproduced, do
not contain printer's markings or have been tampered with in any way, or is sent by postage-due
mail of any kind. Prize claimant may wish to obtain mail insurance. Value
Card Lucky Number materials submitted become the property of the Sponsor will
not be returned. By participating, the participant agrees to abide by and be
bound by these Official Rules. Prizes will be awarded only if participants
comply with these Official Rules. Participant waives the right to assert as a
cost of winning a prize, any and all costs of redemption or travel to redeem
said prize, and any liability which might arise from redeeming, or seeking to
redeem, or from use, misuse, acceptance or possession of said prize or from
participation in prize-related activities. New Hampshire
residents are not responsible for the cost of delivery said prize, if
applicable. All applicable federal,
state and local taxes on all prizes are the sole responsibility of prize
winner. Grand Grand prize
winner will be required to complete and return an Affidavit of
Compliance/Eligibility and Liability/Publicity Release (where permitted) within
fourteen (14) days of date of notification from the Fulfillment Center or prize
will be forfeited and may be awarded to an alternate winner. By accepting a prize, a participant the potential winner agrees to release
The Jimmy Fund, Jiffy Lube, and all other promotional tie-in partners including
Best BuyBest Buy, SuperClubsSuperClubs,
COHEN-FRIEDBERG ASSOCIATES, their respective directors, officers and employees,
affiliates, subsidiaries, distributors, and agents, including but not limited
to advertising and promotion agencies, from any and all liability of any kind
arising from or in connection with any prize, and acknowledges that said parties
have neither made nor are in any manner responsible or liable for any warranty,
representation or guarantee, express or implied, in fact or in law, relative to
any prize including but not limited to its quality, mechanical condition or
fitness for a particular purpose. By accepting a prize, winner agrees to the
use of his/her name and/or photograph in advertising worldwide for this or
similar promotions and grants to Sponsor any and all right to said use, without
further compensation, except where prohibited.

ELIGIBILITY: The
Promotion is offered only to legal residents of the United States of America
("U.S."), residing in the U.S., excluding Puerto Rico, Guam, and the U.S.
Virgin Islands, who are at least 18 years of age at the time of entry. Persons in
any of the following categories are NOT eligible to participate or win prizes:
a) Employees of Jiffy Lube, and of its respective affiliates and
subsidiaries, its participating Franchisees, its advertising and promotion
agencies, and their respective employees; b)
Employees of Dana-Farber Cancer Institute, other promotional tie-in partners
Best Buy and SuperClubs, their respective affiliates, subsidiaries and
distributors; c) Individuals engaged in the development, production or
distribution of materials for this promotiongame;
d) Persons who are immediate family of, dependents of (as defined by IRS
regulations), or who live in the same household as any person in any of the
above categories. Prizes won by minors will be awarded in the name of
the parent or Legal Guardian for the benefit of the minor
Promotion void wherever prohibited by law.
9)
VERIFICATION: Lucky Numbers found on Value Cards are null and
void and will be rejected if not obtained through authorized legitimate
channels; or if illegible, mutilated, torn, altered, counterfeit, photo copied,
mechanically reproduced, tampered with in any way, miscut, blank or partially
blank, misregistered, if they contain unique typographical, mechanical, printing,
production or other errors; or if card fails any validation test. Liability
for the issuance of a Value Card containing printing or other errors, if any,
is limited to the replacement with a Value Card from random stock of Value
Cards while supplies last. Antifraud detection devices will be used to verify
winners. No more than the advertised number of prizes or offers in each prize
category will be awarded. In the event the number of winning submissions
received is greater than the number of prizes or offers provided for in these
Official Rules, only the number of prizes indicated will be awarded based on a
random drawing of all such submissions received at that prize/offer level.

VALUE CARD DISTRIBUTION: Value Cards will be distributed at participating
Jiffy Lube locations in the states of Massachusetts, New Hampshire, Rhode
Island and Vermont while supplies last.
11)
ONLINE GENERAL RULES
& MISCELLANEOUS: Entries made
via the Internet will be deemed made by the authorized account holder of the
e-mail address submitted at time of entry. The authorized account holder is
deemed as the natural person who is assigned to an e-mail address by an
Internet access provider, online service provider or other organization that is
responsible for assigning e-mail addresses or the domain associated with the
submitted e-mail address. Potential winner(s) may be required to provide
COHEN-FRIEDBERG ASSOCIATES, with proof that the potential winner(s) is the
authorized account holder of the e-mail address associated with the potentially
winning entry. Sponsor and COHEN-FRIEDBERG ASSOCIATES are not responsible for
any typographical and/or other error(s) in printing of the offer,
administration of the Promotion or the announcement of prizes or winning Nnumbers.
Verification of prize claims and conduct of all drawings are under supervision
of COHEN-FRIEDBERG ASSOCIATES. Sponsor and COHEN-FRIEDBERG ASSOCIATES are not
responsible for any problems or technical malfunction of any telephone network
or lines, computer online systems, servers, or providers, computer equipment, software,
failure of any e-mail or entry to be received or sent by Sponsor or
COHEN-FRIEDBERG ASSOCIATES, on account of technical problems or traffic
congestion on the Internet or at any web site or by human error which may occur
in the ability to enter a participant's Value Card code in this promotion, or
any combination thereof, or otherwise, including any injury or damage to
entrant's or any other person's computer related to or resulting from
participation or downloading any materials in the promotion. Persons found
tampering with or abusing any aspect of this promotion, as solely determined by
the judges, will be disqualified. If disqualified for any of the above abuses,
Sponsor and COHEN-FRIEDBERG ASSOCIATES reserve the right to terminate entrant's
eligibility to participate in the Promotion. CAUTION: ANY ATTEMPT BY AN
ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE
OPERATION OF THE PROMOTION MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND
SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVE THE RIGHT TO SEEK DAMAGES FROM
ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW. In the event any portion of this promotion is
compromised by virus, bugs, non-authorized human intervention, fraud, technical
failure or other causes beyond the control of the Sponsor or COHEN-FRIEDBERG
ASSOCIATES which, in the sole opinion of COHEN-FRIEDBERG ASSOCIATES, corrupts,
or impairs the administration, security, fairness, integrity or proper play of
the Promotion, Sponsor and COHEN-FRIEDBERG ASSOCIATES reserve the right in
their sole discretion to cancel, modify, suspend or terminate the Internet
portion of the promotion, but may continue with the free, Second Chance means
of entry portion. Sponsor and COHEN-FRIEDBERG ASSOCIATES are not responsible for
any error, omission, interruption, deletion, defect, delay in operation or
transmission, communications line failure, theft or destruction, or
unauthorized access to, or alteration, of entries.

COHEN-FRIEDBERG ASSOCIATES and Sponsor are not
responsible for late, lost, illegible, incomplete, stolen, misdirected, or
postage due mail. Prizes are not transferable, are not endorsable, and cannot
be substituted for cash, except that, in the event that a prize is unavailable
at the time it is awarded, Sponsor reserves the right to substitute a prize of
equal or greater value. Prizes can not
be redeemed for cash value.
13) All taxes, fees, and surcharges on prizes won are
the sole responsibility of the winner.
14) This Promotion is conducted in English (which will be
given its everyday ordinary meaning) and any and all disputes will be resolved
in the appropriate courts in the Commonwealth of Massachusetts in the United
States, exclusively, and subject to the laws of the Commonwealth of
Massachusetts. Should there be a conflict between the laws of the Commonwealth
of Massachusetts and any other laws, the conflict will be resolved in favor of
the laws of the Commonwealth of Massachusetts.
15) WINNERS' LIST will be available after February
14, 2006 by sending a self-addressed stamped envelope by February
28, 2006 to: Jimmy Fund Winners List, PO Box 2253 Framingham, MA 01703-2253.
